No. We are an independent professional document preparation service. We are not affiliated with any government agency. We work on your behalf to submit requests to the appropriate state or county vital records office. You can always request death certificates directly from the government office, but our service handles all the paperwork and logistics for you.
You will need: the full legal name of the deceased, date of death, county and state of death, and the number of certified copies you need. You will also need to provide your contact information and confirm your relationship to the deceased. Some states require additional documentation which we will guide you through.
If we are unable to fulfill your order for any reason within our control, we will issue a full refund. Because we advance official government fees on your behalf, refund requests must be made before your application is submitted to the vital records office. Please contact [email protected] with any concerns.
